Parties & Purpose. By submitting this form and/or participating in an event where recording may occur, I (“Participant”) grant DJ Ultralight Entertainment LLC (“Company”) permission to capture and use my image, likeness, voice, and performance in photographs, video recordings, audio, and other media created at or in connection with the event (the “Media”) for the promotional purposes described below.
Scope of Use. I grant DJ Ultralight Entertainment LLC a perpetual, worldwide, royalty-free, sublicensable, and transferable license to use, reproduce, display, perform, publish, distribute, adapt, edit, and create derivative works from the Media, in any format or medium now known or later developed, including but not limited to DJ Ultralight Entertainment LLC websites, social media channels, online advertising, emails, print materials, trade shows, and publicity.
No Compensation. I understand and agree that I will not receive compensation (monetary or otherwise) for DJ Ultralight Entertainment LLC’s use of the Media and that DJ Ultralight Entertainment LLC has no obligation to use the Media.
No Approval Rights. I waive any right to inspect or approve the Media or the finished product or any accompanying text or context in which the Media may appear.
Release. To the fullest extent permitted by law, I release and hold harmless DJ Ultralight Entertainment LLC and its owners, employees, contractors, agents, licensees, successors, and assigns from any and all claims, demands, and causes of action arising out of or related to the use of the Media as permitted herein, including claims for invasion of privacy, violation of publicity rights, defamation, and copyright/moral rights.
Minors. If the Participant is under 18, consent must be provided by a parent or legal guardian, who represents and warrants that they have legal authority to grant this consent on the minor’s behalf and agrees to these Terms.
Duration & Revocation. This permission is perpetual and may be revoked prospectively (for future use only) by sending a written request to [insert your contact email or contact form URL] with sufficient information to locate the Media. Revocation does not require DJ Ultralight Entertainment LLC to remove or cease uses already made, uses already committed to, or materials already in production or circulation at the time DJ Ultralight Entertainment LLC receives and reasonably processes the request.
Privacy & Data. Contact details submitted with this form are used to document consent and for administrative purposes in accordance with the DJ Ultralight Entertainment LLC Privacy Policy (link on site). Marketing messages require a separate, optional opt-in.
No Exclusivity / No Obligation. Nothing in these Terms obligates DJ Ultralight Entertainment LLC to produce or publish the Media. Participant retains the right to use their own photos/videos subject to venue or host rules.
Event Host Consent (Supplemental). If you are the Event Host/Organizer, you represent that you will notify attendees of recording at the event (e.g., signage) and that you have necessary permissions from the venue for the event’s operation.
Governing Law. These Terms are governed by the laws of the State of Minnesota, without regard to conflict-of-laws principles.
Acceptance & E-Signature. By checking the required boxes and submitting this form, I affirm that I have read and agree to these Terms. My electronic signature has the same legal effect as a handwritten signature to the fullest extent permitted by law.
